What makes a medical certificate valid in India?

A valid Indian medical certificate is a document issued by a registered medical practitioner certifying that they have personally examined the patient on a specified date. It is recognised by employers, schools, government departments, insurance companies, and courts only when it carries the practitioner's original signature and the official clinic or hospital stamp. A draft template — like the PDFs produced by this tool — is a formatting aid, not a legal document.

The legal authority to issue medical certificates in India flows from the registration system maintained by the National Medical Commission (NMC) for Allopathic doctors (M.B.B.S. and above), and by the corresponding statutory councils for AYUSH practitioners — the Central Council of Indian Medicine for Ayurveda, Unani, and Siddha; the Central Council of Homoeopathy for Homeopathy; and the equivalent council for Yoga and Naturopathy. State Medical Councils such as Maharashtra Medical Council (MMC), Karnataka Medical Council (KMC), Tamil Nadu Medical Council (TNMC), Delhi Medical Council (DMC), and Punjab Medical Council (PMC) issue prefixed registration numbers that you can typically verify on the council's public register.

Fields HR teams and academic offices check

  • Doctor name with title and qualification. “Dr.” prefix followed by full name, with qualifications (M.B.B.S., M.D., D.O.M.S., B.D.S., M.D.S., and so on) printed directly under the name.
  • Registration number. Typically formatted as a state council prefix followed by a serial number — for example, MMC-54321 (Maharashtra), KMC-48291 (Karnataka), DMC-67890 (Delhi), TNMC-43820 (Tamil Nadu), KSMC-30218 (Kerala), or PMC-77120 (Punjab). The NMC itself does not issue a national number on most certificates; the state council number is what HR validates.
  • Clinic or hospital name, address, and stamp. The rubber stamp or seal is the single strongest visual authentication signal in Indian HR practice. Certificates without a stamp are rejected more often than those with any other defect.
  • Specific diagnosis.“Acute viral fever,” “migraine,” “acute gastroenteritis” — concrete enough to be evaluated. Vague entries such as “weakness” or “not feeling well” are commonly rejected.
  • Rest period with start and end datesthat match the actual absence. For sick leave certificates the verbatim CAG Form 4 phrasing is “a period of absence from duty of [N days] with effect from [date] is absolutely necessary for the restoration of his/her health.”

Indian sick leave law in one paragraph

Private-sector sick leave entitlement is regulated at the state level under each state's Shops and Establishments Act — Maharashtra, Karnataka, Tamil Nadu, Delhi, Gujarat, Telangana, and so on each have their own statute. Most permit 5 to 12 days of paid sick leave per year, with a medical certificate required for absences exceeding two or three days. Government servants are covered by the Central Civil Services (Leave) Rules, 1972 and equivalent state rules such as Kerala Service Rules Part I, Rule 117, which prescribe CAG Form 4 for non-gazetted officers and CAG Form 3 for gazetted officers. The Employees' State Insurance Act, 1948 provides cash sickness benefit to insured workers earning below the prescribed threshold; ESI claims require an Insurance Medical Officer's certificate (Form 9, 12 or 16 depending on stage).

Why forgery matters — Bharatiya Nyaya Sanhita 2023

On 1 July 2024 the Bharatiya Nyaya Sanhita (BNS) 2023 replaced the Indian Penal Code (IPC) 1860 as India's primary criminal statute. The BNS retains the offence of forgery and use of forged documents, with penalties extending to seven years of imprisonment for fraudulent use of a medical document. Submitting a draft template to HR, a school, an insurer, or a court as if it were a real certificate — without the issuing practitioner's examination and original signature — is a punishable offence under the BNS.
