What is a doctor's note for work?
A doctor's note is the shortest and most common form of medical documentation in India — typically used for absences of one or two working days. Unlike a formal medical certificate (which usually requires the patient's signature at the top and detailed certifying phrasing), a doctor's note is a brief third-person attestation that the patient was seen by the doctor on a specific date. In Indian workplaces, doctor's notes are most often used for sudden one-day absences (food poisoning, migraine, viral cold, menstrual pain), follow-up half-day visits, or recurring conditions where the employee already has an HR-approved chronic-illness file on record.
Why doctor's notes are different from full medical certificates
Doctor's notes are intentionally simpler than the verbatim CAG Form 4 or Kerala Rule 117 sick leave certificate format: • They typically do not include the patient's signature at the top. The Indian government convention of "whose signature is given above" is reserved for formal sick leave certificates requiring multi-day absences. • They are written in the third person — "This is to certify that [patient] consulted Dr. [doctor] on [date]" rather than the first-person "I, Dr. [doctor], hereby certify…" • They do not always include a specific diagnosis. A general phrase such as "acute viral illness" or "medical reason" is often acceptable for short absences. • They are commonly issued on a clinic's prescription pad rather than on official certificate stationery. Despite this informality, a doctor's note still requires the doctor's registration number and clinic stamp to be valid for HR submission.
When a doctor's note is sufficient — and when it isn't
A doctor's note works well for: • Short absences of 1 to 2 working days where the HR threshold for a formal certificate has not been crossed. • Half-day or partial-day medical visits during work hours. • Cases where the employee returns to work on the same day with a note confirming they were seen. • Recurring conditions where the employee already has a documented chronic-illness exemption. A doctor's note is generally NOT sufficient for: • Absences longer than 2 to 3 days — most state Shops and Establishments Acts and Central Civil Services Rules require a formal medical certificate for extended absences. • ESI (Employees' State Insurance) benefit claims — these require a Form 9 / Form 12 certificate from an ESI-empanelled doctor. • Government servants on leave under CCS Rules — Form 3 (gazetted) or Form 4 (non-gazetted) is required for absences of more than one day. • Maternity benefit claims — Form B (Maternity Benefit Rules 1961) is required. • Insurance claim documentation, which typically requires a detailed diagnosis, treatment record, and prescription.
