When students need a medical certificate in India

Indian schools and universities typically require a medical certificate when a student misses three or more consecutive days of class, or when a student misses an examination on medical grounds. The specific threshold varies — some institutions require certification for any absence; the IITs and NITs require formal documentation for any missed exam regardless of duration; central universities and state boards often set the bar at three to five working days. Medical certificates for students are also required for one-time events such as admission to undergraduate programmes (Joint Seat Allocation Authority for IIT/NIT/IIIT requires the JoSAA Business Rule Annexure 7 medical fitness format), hostel allotment, sports participation, study-abroad applications, and re-evaluation requests after a missed examination.

The NIT Calicut verbatim student medical certificate format

Most Indian central and state universities follow a format closely modelled on the NIT Calicut prescribed student medical certificate. The verbatim certifying phrasing is: "I, Dr. [Name], after careful personal examination of the case, hereby certify that [Student Name], whose signature is given above, is suffering from [Diagnosis]. He/she was not in a condition to attend class / write the examination during the period from [from-date] to [to-date] ([N days])." Note two important differences from a workplace sick leave certificate: • The header field is "Name of the student" (not patient) — this aligns with academic record-keeping conventions. • The duration phrasing is "was not in a condition to attend class / write the examination" rather than "absence from duty of N days." This explicitly distinguishes class absence from exam absence, which may have different academic consequences (attendance shortage vs. exam re-evaluation).

Fields academic institutions check

When an academic institution validates a student medical certificate, the registrar or warden typically checks: • Student's full name as registered with the institution. The name on the certificate must exactly match the name on the institute's records. • Roll number, registration number, or admission ID — although the verbatim NIT format does not include a roll number field, most institutions ask the student to write it on the certificate at submission time or attach a separate ID copy. • Class, year, programme, and section — usually written below the name field. • Doctor's NMC or State Medical Council registration number, with the institutional health centre stamp if issued by the campus medical officer, or with the external clinic stamp if issued by an outside doctor. • Specific dates of absence with the exact phrasing "attend class" and/or "write the examination" depending on what the student missed. • Clear diagnosis — "acute gastroenteritis," "viral fever with body aches," "migraine." JoSAA and other admission medical certificates

The Joint Seat Allocation Authority (JoSAA), which conducts admissions to IIT, NIT, IIIT, and certain centrally-funded technical institutes, requires a specific medical fitness certificate at the time of physical reporting. The format is published in the JoSAA Business Rule document, typically as Annexure 7. It differs from the routine student sick leave certificate in two ways: • It is a fitness certificate (affirmative — "found medically fit to pursue the programme") rather than a sick leave certificate (negative — "unable to attend"). • It typically includes a vital signs section (height, weight, BP, vision, hearing, blood group) and a declaration that the candidate is free from any contagious disease. Re-evaluation and attendance shortage cases

Two specific scenarios where a student medical certificate has high stakes: • Examination re-evaluation: Most universities allow a student who missed an examination on medical grounds to apply for a make-up examination or re-evaluation. The application must usually be submitted within 7 to 15 days of the missed exam, accompanied by the medical certificate. The certificate's date range must include the date of the missed exam. • Attendance shortage: Most Indian universities require 75% minimum attendance. If a student's attendance drops below 75% due to medical absences, they may be barred from sitting for end-semester exams. Does the certificate need to mention the missed exam by name?
